Regulation of Follicle Development and its Clinical Implications

Assisted reproductive technologies include ovarian stimulation in the majority of the cases. In the recent years, experience accumulated in the treatment of infertile couples led to the individualization of ovarian stimulation protocols. Every patient needs to be treated according to strategies developed with the help of basic science research devoted to a better understanding of ovarian physiology. This symposium therefore includes basic science and clinical experience.

In the basic part, we would like to summarize the knowledge on the regulation of follicle development and ovulation based on animal and human studies. The genetic basis of folliculogenesis and its control will be summarized as well as the perspectives of the most recent discoveries. An update on the in vitro growth of follicle will be presented.

In the clinical section, the role of FSH receptor polymorphism will be exposed.The pathophysiology of anovulation will be presented with the latest data on PCOS as well as the stimulation regimens using gonadotrophins or aromatase inhibitors. The effect of age on ovarian function and on oocyte chromosomic abnormalities will be discussed as well as the role of AMH in assessing ovarian reserve. An update on ovarian stimulation for IVF will be presented with the choice of the starting dose, the use of GnRH antagonist, the need for LH activity and the luteal support. Data will be presented on treating patient with medical history or predisposing gene to cancer and on the latest results on imprinting gene defect and ART. A conference on the perspective of stem cells will be given.

OBJECTIVES

After attending this conference, the participants will have an updated knowledge on:

  • The regulation of follicular development and ovulation;
  • Ovarian dysfunction in PCOS and treatments possibilities;
  • The processes of ovarian aging and the use of AMH as an evaluation of ovarian reserve;
  • Ovarian stimulation strategies - gonadotrophin dose, GnRH antagonists, LH supplementation and luteal support;
  • The possibility to treat cancer risk patients and recent data on ovarian cryopreservation.
